It is better to play safe. Since Laos fishes comes from the river where all wastes also go the danger of geeting diarrheal infection is higher. Better cook the fish well than be sorry.

This is not stupid question. For me your question is good for some many Lao they are still understand as your question and that it's misunderstanding.
The food undercook from fresh water fish is harmful your health, you may risk to be infected with many parasites esp. liver fluke parasite and other such as intestinal fluke.
Liver fluke parasite named Opisthorchis viverrini or OV, estmated infected people in Laos about 2 mill. if long time no treatment it may develop to be cancer of bile ducts and gall bladder including change fucntion of liver.
This parasite will die only by heat from 80 C degree, please stop eating raw fish that without enough cook.
In Japan also sometimes there're few cases that were incident infected also but other type of parasite, even Salmon also could risk to be infected parasite if we take the fresh one directly from sea or river.
Below is pict. of OV's live cycle, the metacercariae in flesh of fish will risk to be infected when eat raw fish

thank you for informations, i don't know does the government workhard to promote this kind of disease in Laos, epecially the rural areas, Many people still keep eating their raw fishes, because i think they lack of knowleges and information from the Ministry of health and local health service.
it is necessay to push the media and make a serious promotion regarding the disease and if people keep eating the raw fish and what is gona happen?
Lao people have one of the world's highest rates of bile duct & liver cancer because they eat padek & laab made from raw freshwater fish contaminated with liver flukes. Older Lao nok who spent their younger days in Laos also face this problem, their livers still have the parasites that infected them decades ago when they were kids.
You hardly find this problem in Japan because the raw fish that they eat is mostly from the sea (this parasite lives only in freshwater), & sanitation there is far better.

The fish from the sea also is not 100% safety for eating without undercook if you dont know how to prepare and also not all type of fish could eat as raw. I mention here is not taste that could eat or not but I mean for safety from parasite bcz there are many parasites also infected to fish in the sea then could transfer to and harmful to human, easily know here ex. Salmon (I also mention above)
